Utah Hunter Education Program Duplicate Certificate Affidavit
To request a duplicate Hunter Education certificate, fill out the following form. Once you submit the form, you will receive an affidavit number by email and be automatically directed to a payment portal. At the payment portal, enter the affidavit number you received, then go to the checkout and pay $10.
If you prefer to complete a paper version of this form instead, and/or want to pay using a personal check, money order or cashier's check, please print out and complete the paper version of this form. Instructions for delivery and payment are at the bottom of this page.
Please submit only one duplicate request per person, and note that requests may take up to 10 days for us to receive. Once we have received your payment and verified your Hunter Education completion, we will mail you your duplicate certificate.
